I've heard of it happening, but I never knew anyone that it had happened to until last week. I foget the model number, but it was a Glock and he was shooting reloads. Glock says not to shoot reloads, but "I'd done it hundreds of times with no problem".
His point was: "you never know when you pick up brass if it is yours or not and how many times it has been reloaded."
Another RO that I respect a lot marks each of his cases with a Sharpie so he knows how may times it has been reloaded. If there isn't a mark, he knows that it isn't his.
Moral of the story: be careful especially when the case isn't fully supported.
